As medical devices and information systems become increasingly complex, the issue of how to support users becomes more important. However, many current help systems are often ignored or found to be too complicated to use by clinicians. In this article, we suggest an approach that allows designers to think about user support and automating tasks in ways users find more acceptable. The issue we address in particular is the notion of transparency and to what extent it allows the end-user to understand and critique the advice given. We have found that one central problem with existing support systems is that often the end-user does not understand the differences between the automated parts and the parts that have to be done manually. By taking aspects of transparency and control into account when designing an automated tool it seems that some of the more refractory issues that help systems pose for professional users can be addressed.

Dinka, David, Nyce, James M., Timpka, Toomas and Holmberg, Kristina (2006): Adding Value with 3D Visualization and Haptic Forces to Radiosurgery—A Small Theory-Based, Quasi-Experimental Study. In Journal of Medical Systems, 30 p. 293–301

This study investigates the use of 3D representation and haptic technology in radiosurgery. The users, experienced users of the Leksell GammaKnife, prefer the 3D representation for constructing a first draft of their plan for neurosurgical intervention. When it comes to the final adjustments, however, they still choose the traditional 2D representation and interaction devices. The perceived control over the radiosurgical process is not considered adequate using the 3D interaction methods. Practitioners do not consider the haptic forces implemented in this test system useful. Possible explanations for these findings are discussed in the paper.

Dinka, David, Nyce, James M. and Timpka, Toomas (2005): GammaKnife surgery: Safety and the identity of users. In Technology and Health Care, 13 (6) pp. 485-495

In this study we investigated safety-related usability issues of an advanced medical technology, a radiosurgery system. We were interested in which criteria are important for users when a system's usability and safety is to be improved. The data collection was based on interviews and observations at three different sites where the Leksell GammaKnife® is used. The analysis was qualitative.The main finding was that the user's identity or professional background has a significant impact both on how he or she views his or her role in the clinical setting, and on how he or she defines what improvements are necessary and general safety issues. In fact, the opinion even of users experienced in safety-related problems was highly influenced by how they related to the technology and its development. None of the users actually considered Leksell GammaKnife as lacking in safety, instead, their assessment was directed towards potential future system improvements. Our findings suggest that the importance of user identity or professional background cannot be neglected during the development of advanced technology. They also suggest that the user feedback should always be related to user background and identity in order to understand how important different issues are for particular users.

Timpka, Toomas, Padgham, Lin, Hedblom, Per, Wallin, Stefan and Tibblin, Gosta (1989): A Hypertext Knowledge Base for Primary Care -- Limeds in Lincks. In: Proceedings of the Twelfth Annual International ACM SIGIR Conference on Research and Development in Information Retrieval 1989. pp. 221-228.

In organized health care, primary care is the first level. It is characterized by the wide span of health problems managed as well as remote location from traditional medical information and knowledge sources. The LIMEDS project has formulated the special requirements for integrated knowledge and data base management in primary care. This paper presents Gosta's book, a hypertext knowledge base implemented in LINCKS, an object oriented, networked database system. Firstly, aspects which make integrated hypermedia systems particularly suitable for application in primary health care are explored. We then describe the hypertext knowledge base, consisting of 500 basic text objects and 3000 links, and current implementations using the NODE data model. NODE is implemented on a SUN III fileserver, and the user interface for the hypertext context on Apple Macintosh. Combination of design methods towards a parallel means-ends strategy was found to be necessary to achieve Gosta's book. Design groups need to be composed of computer science, medical, psychological and organizational competences.
